This form may not be reused in any way shape or form due to the fact that students are being evaluated on different criteria. There must be a signature from a lead teacher, director, or supervisor. The second form is a Time Sheet. This is to be filled out and signed by the student and a lead teacher, director, or supervisor. These hours need to be working hours, lunch breaks do not count. Each student must complete a minimum of 45 hours to meet the Practicum Requirement.
